I understand that any and all activities in which I will participate, including but not limited to use of tools (potential injuries may be caused by the use of tools by an individual to themselves, or by the use of tools from others nearby) construction projects, aviary maintenance, aviary cleaning, food and water preparation, bird and animal removal, rescue and transport, landscaping, relocation, and education activities that require varying levels of walking and working on varied terrains and site conditions, some of which are potentially dangerous for people. Any and all activities around wildlife are inherently dangerous and may cause serious or grievous injuries, including bodily injury, damage to personal property and/or death. On behalf of myself, my heirs, assigns, administrators, executors and next of kin, I waive all claims for damages, injuries and death sustained to me or my property that I may have against Arizona Wildlife Resource, LLC. This includes claims in tort, contract, equity or otherwise. I further agree and warrant that if at any time I believe conditions to be unsafe, I will immediately discontinue further participation in the volunteer activity.

I am aware that my choice to participate in any and all Arizona Wildlife Resource, LLC activities may place myself in potentially dangerous or risky situations, including but not limited to the following: forces of nature, injuries that may be associated with walking and working in uneven rugged terrain, working next to, and inside of, open trenches, breathing dust, exposure to plants, animals, insects and bites or stings from insects, use of tools, construction activities, lifting rocks, and traveling by automobile to and from any site(s).

I understand that despite the safety protocols undertaken by Arizona Wildlife Resource, LLC, I affirm that my participation is my own choice and I assume full responsibility for such dangers and risks and I expressly release and will hold harmless Arizona Wildlife Resource, LLC, Arizona Wildlife Resource, LLC employees, volunteers, and staff from any and all liability actions, causes of action, debts, claims or demands which may arise from participation in this program. I agree to indemnify and hold harmless Arizona Wildlife Resource, LLC for all special expenses incurred in connection with any injury or danger suffered by me.

The provision of this assumption of risk will continue in full force and effect even after the termination of the activities conducted by, on the premises of, or for the benefit of Arizona Wildlife Resource, LLC, whether by agreement, by operation of law, or otherwise.

This signed agreement shall constitute a release and assumption of risk by me, and shall be binding on my heirs, executors, and administrators.

Photo Release Agreement

I hereby grant Arizona Wildlife Resource, LLC permission to use my likeness in a photograph, video, or other digital media (“photo”) in any and all of its publications, including web-based publications, without payment or other consideration.

I understand and agree that all photos will become the property of the Arizona Wildlife Resource, LLC and will not be returned.

I hereby irrevocably authorize the Arizona Wildlife Resource, LLC to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo.

I hereby hold harmless, release, and forever discharge the Arizona Wildlife Resource, LLC from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

I HAVE READ AND UNDERSTAND THE ABOVE PHOTO RELEASE. I AFFIRM THAT I AM AT LEAST 18 YEARS OF AGE, OR, IF I AM UNDER 18 YEARS OF AGE, I HAVE OBTAINED THE REQUIRED CONSENT OF MY PARENTS/GUARDIANS AS EVIDENCED BY THEIR SIGNATURES BELOW. I ACCEPT:
